Consent to Telehealth Services
This is an informed-consent document. It describes how remote care through Chubby No More Inc. works, what it can and cannot do, and the choices available to you. You will be asked to acknowledge it before submitting your clinical intake.
1. What telehealth means
Telehealth is the delivery of healthcare using communications technology instead of an in-person visit. It may include reviewing written health information, secure messaging, telephone, or video.
2. Synchronous and asynchronous care
Synchronous care happens in real time, such as a live video or telephone visit. Asynchronous care means you submit information and our licensed provider reviews it later. Chubby No More Inc.'s clinicians primarily deliver care asynchronously through your written intake and secure messaging, and may offer live video or telephone visits where clinically appropriate or required by your state.
3. Potential benefits
- Access to our licensed provider without travel to a clinic.
- More flexible scheduling and faster initial review.
- Greater privacy and convenience for follow-up.
- Continuity of monitoring through structured check-ins.
4. Potential risks and limitations
- Information you provide may be incomplete or inaccurate, which can affect clinical judgment.
- A condition may be missed that an in-person examination or testing would identify.
- Delays in review or response can occur.
- Treatment may be less appropriate for you than in-person care in some situations.
5. No complete physical examination
Your provider cannot perform a hands-on physical examination remotely. Some findings, such as certain abdominal, cardiac, or neurologic signs, can only be assessed in person.
6. Technology failure
Connectivity problems, device failures, or software issues may interrupt or delay care. If a session fails or a message does not go through, contact our support team at hello@chubbynomore.com or (908) 842-3447, Monday through Friday, 9:00 a.m. to 6:00 p.m. ET, and we will help you reschedule or resubmit your information.
7. Privacy and security limitations
Reasonable safeguards are used, but no electronic communication is completely secure. Risks include interception, misdirected messages, and unauthorized access to your device. Use a private device and network when possible.
8. Alternative care options
You may choose in-person care with a local clinician instead of telehealth at any time. Choosing not to use telehealth does not affect your right to seek care elsewhere.
9. Your right to ask questions
You may ask questions about your evaluation, medication options, risks, alternatives, and costs at any point, and you may request that a question be answered before you proceed.
10. Your right to withdraw consent
You may withdraw this consent at any time without affecting care you have already received. Withdrawal may end your ability to receive further telehealth services. To withdraw, email hello@chubbynomore.com or call (908) 842-3447, Monday through Friday, 9:00 a.m. to 6:00 p.m. ET.
11. Emergency limitations
Telehealth is not for emergencies. If you have severe or sudden symptoms, call 911 or go to the nearest emergency department. For mental-health crisis support in the United States, call or text 988.
12. You must disclose your physical location
You must tell your provider the state where you are physically located at the time of care. Licensure rules depend on your location, and care may not be lawful if your location is misreported.
13. Provider licensure and state availability
Care is provided directly by the licensed clinicians employed by Chubby No More Inc., each of whom is licensed in the state where you are physically located at the time of your visit. Services are offered only in states where our clinicians hold an active license, and availability is confirmed for your state during the eligibility quiz and intake.
14. Electronic communications
You consent to receive care-related communications electronically, including messages about your evaluation, prescriptions, and follow-up, where permitted by law.
15. Prescriptions are not guaranteed
Consenting to telehealth does not entitle you to any medication. Your provider may decline to prescribe, may prescribe something different from what you expected, or may recommend non-medication approaches. If your provider declines to prescribe, you are charged $0 and any payment pre-authorization is released.
16. In-person referral when appropriate
If your provider determines that in-person evaluation, laboratory testing, or specialist care is needed, they may refer you and may decline to continue remote treatment until that care occurs.
17. Documentation in the medical record
Your consent, intake responses, and clinical communications are documented in your medical record, which Chubby No More Inc. maintains as the clinical entity providing your care.
18. State-specific consent supplements
Some states require additional consent disclosures or a specific consent format. Where your state requires it, we present the required supplemental disclosure to you during intake, and your acknowledgment of that disclosure is recorded alongside this consent.
19. Electronic signature
By typing your full legal name and submitting your intake, you sign this consent electronically and agree that your electronic signature has the same effect as a handwritten signature. Each acknowledgment records: intake or patient ID, full name, state, consent version, timestamp, electronic signature, and withdrawal status.
20. Effective date and consent version
Consent version: 1.0. Effective date: August 18, 2026. Approving clinical reviewer: the Chubby No More Medical Director, reviewed August 18, 2026, with the next scheduled clinical review on February 18, 2027.
